what would be better, extended studs and lug nuts, or just Tuner open ended lug nuts?

You shouldn't need longer studs anyway. What kind of wheels are they?
Stock lugs will almost never fit, you need tuner lugs. Go to Canadian Tire and get the set of McGard Splinedrive tuner lugs, M12x1.5
