What are you looking for stock, p&p head, or a new head (reman). Check out the classifieds or regional forums for a stock on. Seen a few around there

Doesn't exist. There is a company that has been selling race prepped LD9 heads for a while, but they don't use a special casting. It's a stock head that's ported and has upgraded valvetrain. They use dual valve springs rated to 10,000 RPMS, so I really wouldn't recomend it for anything other than full-race. Race LD9 heads

Otherwise, your options are
A) port and polish the head yourself or by a shop
B) modify a Quad4 head to fit to the LD9 block.

what normally fails on the 2.4 liter cylinder heads?

gasket failure. and or cracked head.
