recently, on my 91 chevy 454ss the engine coolant line had a leak in it and got all over my belt, this caused it to come off while i was driving. well i fixed the leak and replaced the belt and ever since then its been giving me problems! the belt came off today for the 3rd time this month! and for some reason, when the belt is on, my gas pedal starts to vibrate when im going between 55 and 60 mph. it has never done that before and today it was doing it and while i was driving the belt came off again and im getting fed up with it. i tightened the pully on the alternator because it was a little loose but i cant see why else it would keep coming off?????
Has it happened since you tightened the pulley?
I don't know much about the 454, but are you sure you have the belt strung the right way? Depending on where your tensioner is, it may be possible that you can run the belt a couple different ways. If you can, try both, and see which one is tighter. The only other thing I can think of is maybe you aren't getting the belt in the pulley grooves well enough, where the belt starts tracking the next rib over, until the belt "pulls" itself off the pulley.
Other than that, I've got nothing, because I can't imagine even very harsh vibrations setting your belt free like this.

yea maybe it was put back on wrong because it probably can be put on more ways than one. but yea it comes off really easily. i just cant quite figure it out
by the way, im tryin to put the belt back on myself this time. i cant get it to fit on though, is there something i have to loosen up or some trick to put the belt back on. i have a book that shows how the belt should be but now i just need to put it on!!!
If you're belt looks like this...
click...then the tensioner is the small pulley on the left side and/or the small white pulley in the center at the top. It's hard to tell, because both are touching the back of the belt.
Use a wrench that fits the nut on the tensioner (probably around 5/8") and when you turn it, it will force the tensioner back. Slip the belt over the tensioner, and slowly let the pulley tighten against the belt. Hope that helps.

This may help... a club for Chevy bigblock engines. It would prolly' work out better than posting on a j-body website. Not many here have a 454 in their cavalier.
