where do you get the threaded shift knobs for the b&m shifters or the fidanza shifter

Look HERE! That link will take you to the B&M manual shifter accessory page. the 2nd thru 5 knob will fit 00-05 B&M short throw (and probally the fidanza shifter)

Then E-mail Jim at Gravanatuning, Jason at A+ or Karo at Car Customs. All 3 can get those knobs for you.

You can also order those knbs fro Autozone, but they WILL be more expensive than if you got it from one of those 3 mentioned above.
